Abstract: Aortitis and aortic valve insufficiency are referred to as extraskeletal manifestations of ankylosing spondylitis (AS). Torpid course of diseases is the reason for late seeking medical care at the stage of irreversible changes and severe dysfunction of the cardiovascular and musculoskeletal systems. The article presents a clinical case of successful surgical treatment of severe aortic valve insufficiency in a patient with late diagnosed AS. The problems of cardiovascular pathology in AS, interdisciplinary interaction of doctors, choice of antirheumatic therapy and perioperative management of a very high cardiovascular risk patient with AS are discussed.
