Abstract: Sepsis frequently serves as the proximate factor contributing to mortality in critically ill patients in intensive care units. This study was to investigate the clinical significance of miR-4638-5p in patients with sepsis and its role in regulating LPS-induced inflammatory response. The study included 149 patients with sepsis with a mean age of 54.97 ± 8.57, including 86 men and 63 women. In addition, 98 healthy controls with a mean age of 53.63 ± 11.51 were included, including 52 male and 46 female patients. The expression level of miR-4638-5p was detected by RT-qPCR, and the diagnostic role of miR-4638-5p was analyzed using ROC curve. In addition, the levels of TNF-α, IL-6, and IL-1β in the serum of patients with sepsis were detected by ELISA kit. Cell viability was measured using CCK-8 and the apoptosis rate was measured by flow cytometry. This study found that miR-4638-5p was overexpressed in individuals with sepsis, particularly in those who did not survive, and it exhibited diagnostic value for sepsis while also could forecast unfavorable outcomes in sepsis patients. Furthermore, miR-4638-5p was found to modulate the inflammatory response induced by LPS. In conclusion, miR-4638-5p is closely related to sepsis and regulates the inflammatory response in patients with sepsis, and miR-4638-5p may be used as a clinical biomarker for sepsis.
