Abstract: Gallbladder adenomyomatosis (GA) is a benign, acquired pathology of the gallbladder, typically characterized by a favorable prognosis. While patients with GA are generally asymptomatic, nonspecific abdominal pain localized to the right upper quadrant and epigastrium is the most frequently reported symptom. We herein present a case of diffuse gallbladder adenomyomatosis that presented as acalculous cholecystitis , characterized by right upper quadrant pain, fever, and elevated inflammatory markers. Although ultrasound and CT scan did not reveal any radiological abnormalities at initial presentation, a subsequent MRCP confirmed the presence of diffuse gallbladder adenomyomatosis.
