Hospital femoral neck fractures in patients of the older age group with comorbid neurological pathology

Year: 2025
Type: article
Abstract: The risks of acetabular fractures during hospital treatment in patients of the older age group correlate with those during outpatient or home care. The neurological patients have a significant ability to injury. Every year, about 1,700,000 cases of proximal femur fractures are recorded worldwide. The risk of fracture in the acetabular region during the life is about 6 % in men and reaches approximately 18 % in women. Mortality within one year after injury reaches 56.3 % under conservative treatment. Femoral head fractures are usually systematized according to the AO Fracture and Dislocation Classification. Our statistical data for almost twenty years of practical experience allows us to analyze the most frequent causes of hospital femoral neck fractures. We should distinguish, first of all, insufficient control of motor, postural and coordination disorders by nurse-caregivers who provide professional care for the patient. The most objective scale to assess the risk of falling is the Morse Fall Scale. The article presents a clinical case of the hospital femoral neck fracture in a patient of the older age group with comorbid neurological pathology. In order to reduce the risk of patients’ falling with an unpredictable outcome during hospital stay, we recommend the following preventive measures: mandatory testing each patient for the risk of a possible fall; with an average and high risk of falling — providing the patient with an individual nurse-caregiver station; equipping the premises of the medical facility with all necessary technical means for the patient’s safe stay; in case of a patient’s falling — immediately implement the clinical protocol with the involvement of relevant services.
