Synchronous Resection of Gist in the Rectum and Prostatic Adenocarcinoma With the Da Vinci Robotic Platform

Study design: Case Report
Abstract: The synchronous robotic resection of a gastrointestinal stromal tumor (GIST) in the rectum and prostatic adenocarcinoma is a rare and challenging procedure due to the anatomical complexity and the differences in surgical approaches required for each condition, with distinct robotic docking and the proximity of the vesico-urethral anastomosis to the colorectal anastomosis. This case report describes a 65-year-old male patient simultaneously diagnosed with rectal GIST and prostatic adenocarcinoma. The patient had no rectal symptoms, and the diagnosis was made during a Multiparametric MRI for prostate tumor evaluation. The diagnosis was confirmed through ultrasound-guided biopsy. The surgical approach was carried out on the Da Vinci SI platform, involving radical prostatectomy for the prostatic adenocarcinoma, followed by marginal resection of the GIST from the anterior surface of the rectum within the same docking, as demonstrated in the video. The patient had a favorable postoperative course, with discharge on the 3rd postoperative day. This video highlights the importance of meticulous surgical planning and a collaborative approach between specialties for the effective management of complex synchronous tumors.
