Abstract: Looser’s zones are also known as cortical infarctions, Milkman lines, or pseudo fractures. Looser’s zones are transverse lucencies with sclerotic borders which are perpendicular to the cortex traversing partway in the bone. They are associated with multiple etiologies, if bilaterally symmetrical then the likely diagnosis is osteomalacia. Such lesions are also called insufficiency fractures. Vitamin D deficiency may lead to defective mineralization of bones. The patients present with symptoms of pain, limping, and unable to bear weight X-ray of the affected bone can present as pseudofracture or Looser’s zone. Vitamin D deficiency is commonly due to poor diet, lack of exposure to sunlight, and in growing age. Here, we report a case of osteomalacia with Looser’s zone in the medial side of femoral neck bilaterally symmetrical in a young 16-year-old female presenting with pain in both hip joints and limping for 15 days.
