Abstract: Acquired immune deficiency syndrome (AIDS) not only attacks the human immune system but also causes metabolic disorders. Highly active anti-retroviral therapy (HAART) is the main treatment for AIDS, but it can also affect the human metabolic system and cause metabolic disorders. Several studies have been conducted on the effects of HAART on human metabolism, but research on specific treatment regimens is lacking. Therefore, this study explored the effect of the specific treatment regimen of Lamivudine, Tenofovir, and Efavirenz, which is the most commonly used treatment regimen in China. Most of the metabolomic changes in human immunodeficiency virus (HIV)-positive and HIV-negative participants are related to lipids. Therefore, this study focused on the changes in the lipid metabolic profile. This study adopted an untargeted metabolomics approach based on ultra performance liquid chromatography–mass spectrometry (UPLC-MS) to analyze the plasma samples of healthy volunteers, untreated HIV-infected individuals, and treated HIV-infected individuals. The results of the multivariate statistical analysis showed significant differences in metabolomic profiles between healthy volunteers and untreated HIV-infected individuals, as well as between healthy volunteers and treated HIV-infected individuals. The results also showed the effects of HIV-positive participants and the specific treatment regimen on the plasma lipidome.
