Background: Deoxycorticosterone-producing adrenal tumors area very rare cause of mineralocorticoid excess. They are usually malignant butmay be benign. We present a case of a benign deoxycorticosterone-producingadrenal adenoma. Clinical Case: A 42-year-old man with a history of hypertensionon amlodipine and chronic hepatitis B presented with an incidental adrenalnodule on abdominal ultrasound. He had no Cushingoid features. CT with andwithout contrast revealed a 3.2 cm right adrenal nodule measuring 21 Hounsfieldunits on non-contrast series and an absolute washout 61%, consistent with abenign adenoma. The left adrenal gland was normal. Biochemical workup revealed suppressedrenin (0.09 ng/mL/h, ref 0.25-5.82 ng/mL/h) and low-normal aldosterone (5 ng/dL,ref Conclusion: For patients with an adrenal nodule and signsof mineralocorticoid excess including hypertension, hypokalemia and suppressed renin, but normal aldosterone, the possibility of excess production of a non-aldosteronemineralocorticoid should be considered. Workup should include evaluation of DOCand other mineralocorticoid precursors. The mechanism preventing elevation of aldosteronedespite CYP11B2 tumor expression requires further investigation.
