Study design: Clinical Trial
Abstract: Objective: To identify efforts to prevent recurrence of symptoms in CHF patients by using the nursing care assessment approach, Orem's self-care and Kolcaba's comfort theory. Furthermore, providing health education related to CHF management to measure the level of knowledge. Theoretical Framework: This study integrates Orem's Self-Care Theory and Kolcaba's Comfort Theory to identify knowledge and prevention levels in patients with Congestive Heart Failure (CHF). Orem's theory is applied to enhance patients' self-care abilities through education and preventive actions. Meanwhile, Kolcaba's theory is used to assess the impact of interventions on patients' comfort across physical, psychological, social, and environmental dimensions. Method: This study employed a sequential exploratory mixed-method design, beginning with a qualitative phenomenological approach through in-depth interviews and observations using Orem's self-care and Kolcaba's comfort assessment tools on 12 informants and 2 key informants. In the quantitative phase, a pre-experimental (pre-post test) design was used to assess the impact of health education via the "Health Management of Congestive Heart Failure" book on 32 CHF patients over a 3-month period at Siti Fatimah Az-Zahra and Siloam Sriwijaya Hospitals in Palembang. Results and Discussion: The study identified 16 themes related to CHF patients, including symptoms, disease history, family support, coping efforts, relapse triggers, emotional and economic factors, and perceived comfort. The Wilcoxon test showed a significant effect (p = 0.000 < 0.05), indicating that health education using the book "Health Management of CHF Patients" effectively improved patient knowledge. Research Implications: This study highlights important implications for nursing practice, education, and future research by integrating Orem’s and Kolcaba’s theories to support comprehensive, patient-centered care for CHF patients. The findings can guide the development of educational and health promotion strategies that enhance self-care, comfort, and long-term outcomes. Originality/Value: This study uniquely combines Orem’s Self-Care Theory and Kolcaba’s Comfort Theory to address both the functional and emotional needs of CHF patients. By emphasizing patient empowerment and holistic care, it offers valuable insights for developing personalized, evidence-based nursing interventions to improve outcomes in chronic care settings.
