Emerging Pharmacological Interventions for Refractory Rapid Cycling Bipolar Disorder: New Frontiers in Treatment

Year: 2025
Type: article
Study design: Systematic Review
Abstract: Rapid-cycling bipolar disorder (RCBD) represents a severe subtype of bipolar disorder, distinguished by the occurrence of a minimum of four mood episodes each year. This condition results in considerable functional impairment and demonstrates resistance to conventional treatments. Frequently utilized mood stabilizers, including lithium and valproate, often prove ineffective, underscoring the necessity for novel pharmacological interventions. This systematic review aims to assess the efficacy and safety of new therapies for refractory RCBD. Methods We reviewed randomized controlled trials, cohort, observational, and cross-sectional studies on treatments for adults with RCBD, searching MEDLINE, Web of Science, and ClinicalTrials.gov. Primary outcomes included symptom remission and adverse effects, and secondary outcomes were treatment adherence and quality of life. Results Eight studies on levothyroxine, aripiprazole, long-acting risperidone, chromium, ethyl-eicosapentanoate, fluoxetine, mexiletine, and choline were analyzed. Levothyroxine and aripiprazole significantly reduced depressive episodes and prevented relapses. Long-acting risperidone improved treatment adherence, while chromium's effectiveness was undermined by high dropout rates. Ethyl-eicosapentanoate, fluoxetine, and mexiletine offered no notable benefits. Methodological issues and high dropout rates were common across studies. Conclusion Levothyroxine and aripiprazole show promise for refractory RCBD, though alternative treatments lack clarity. Larger, rigorous clinical trials are needed to validate these findings.
