Abstract: Background: Atopic dermatitis (AD) is a chronic inflammatory skin disease that typically manifests in early childhood. It is characterized by eczematous lesions and intense itching, which significantly affect daily functioning. Despite the availability of various treatments, including biologics, AD remains a therapeutic challenge. Tralokinumab (Adtralza ® ) has been available in Poland under the B.124 Drug Program (“Treatment of patients with severe atopic dermatitis”) since November 2023. The study presents the cases of four patients who were the first to be qualified for tralokinumab treatment within the B.124 Drug Program at the Department of Dermatology and Venereology, Medical University of Lodz and the effects of the therapy. Case report: The patients, adults with severe AD since childhood, had previously received unsuccessful treatments. They began tralokinumab therapy under the B.124 Drug Program. At weeks 16 and 28, improvements in skin lesions, itching, and quality of life were observed. No adverse effects were reported, and laboratory tests showed no significant abnormalities. Conclusions: Tralokinumab is an effective treatment for severe, treatment-resistant AD, though longer-term observation is needed.
