Primary Ovarian Leiomyoma: A Common Tumor at an Uncommon Site

Year: 2025
Type: article
Study design: Case Report
Abstract: Introduction: Leiomyoma, the most common benign mesenchymal tumor of the uterus, can develop wherever smooth muscle cells exist, though extrauterine occurrences are rare.Among these, ovarian leiomyomas account for only 0.5-1% of all benign ovarian tumors.Case presentation: This report details a case of ovarian leiomyoma in a 71-year-old postmenopausal woman who presented with abdominal pain, a large abdominopelvic mass, and grade 3 uterovaginal (UV) prolapse.Radiological imaging indicated torsion of a pedunculated fibroid or adnexal mass.Surgical intervention, including laparotomy, hysterectomy, and bilateral salpingo-oophorectomy, confirmed an ovarian leiomyoma through histopathology and immune-histochemical analysis. Conclusion: This case underscores the importance of histopathological and immune-histochemical examinations in diagnosing primary ovarian spindle cell tumors.
