Abstract: . Objective. To analyse the indicators of blood coagulation and anti-coagulation systems in inflammatory and traumatic injuries of the pleural cavity in extreme conditions in order to correct the treatment process. Materials and methods. The medical records and disease course of 79 patients hospitalised for pleural surgery in 2018–2023 were analysed. In the course of standard examinations, radiation diagnostic methods, clinical and biochemical blood tests that could affect the state of the coagulation and anti-coagulation systems (level of total blood protein and its fractions, cholesterol, lipids, C-reactive blood protein, creatinine and blood urea, ferritin, liver samples) were used, and coagulation parameters were studied. Results. The analysis of coagulation parameters showed that in patients with the first manifestations of the inflammatory process of the pleural cavity, the coagulation system was activated, which eventually transformed into the activation of the anticoagulation system. Conclusion. In all patients, the inflammatory pleural process is accompanied by activation of the coagulation system from the first manifestations of its development. Subsequently, this leads to the activation of anti-clotting mechanisms, which should be classified as a sanogenic reaction aimed at preventing thrombosis or dissolution of already formed thrombi of various localisations.
