Conus medullaris syndrome following abdominal aortic aneurysm repair

Abstract: Conus Medullaris is the lowermost end of the spinal cord. Conus Medullaris Syndrome (CMS), a subset of spinal cord ischemia (SCI) has been observed as a clinical presentation and complication of aortic pathology. Incidence of SCI after abdominal aortic surgery ranges from 0.16% to 2.31%. Predisposing factors include low-lying greater radicular artery, prolonged aortic cross-clamping, peri-operative hypotension, embolization and deficient pelvic circulation. In this paper, we present our experience with two patients who developed CMS following abdominal aortic aneurysm (AAA) repair and their clinical course. We also review the existing literature to further our understanding of this rare occurrence, the outcomes of follow-up, early detection, and possible therapeutic options.
