Abstract: Diabetes Mellitus is a major public health challenge that requires continuous care, especially for vulnerable patients. In this context, the Home Care Program stands out as a strategy to ensure continuity of care, promote autonomy, and reduce hospitalizations. This study aimed to demonstrate the role of the Home Care Program in the care provided to patients with Diabetes Mellitus. This is an integrative literature review, with a search for articles in the databases of the Virtual Health Library (VHL), Scientific Electronic Library Online (SCIELO), Latin American and Caribbean Literature in Health Sciences (LILACS), and the National Library of Medicine (PubMed/Medline), covering studies published between 2021 and 2024. The main findings indicated that home care for diabetic patients contributes to effective glycemic control, with reductions in HbA1c and improvements in other clinical and functional parameters such as metabolic profile, blood pressure, self-care, and reduced healthcare costs. Positive aspects were also highlighted regarding care safety, effectiveness for children and the elderly, and health education. However, organizational limitations and inequalities in access and health outcomes were identified. Home care has proven to be an effective strategy for diabetes management, providing clinical and social benefits, especially among vulnerable populations. The results reinforce the importance of a multidisciplinary team and the continuity of care in the home setting. However, further advances are needed to overcome inequalities and structural weaknesses.
