Functional and Clinical Outcomes of Transtibial versus Transportal Techniques in Arthroscopic Anterior Cruciate Ligament Reconstruction Using a Quadrupled Hamstring Graft: The South Indian Experience

Year: 2025
Type: article
Abstract: CONTEXT: There has been an increase in the incidence of ligament injuries of the knee owing to sporting events and traffic accidents. Anterior cruciate ligament (ACL) is the most common ligament injury of the knee. According to the literature, 85% of the tears noted are complete. The commonly used technique in reconstruction is the transtibial technique. Still, studies have shown a nonanatomical placement of the femoral tunnel resulting in rotational instability of the knee. This study was undertaken to compare and analyze the two techniques and also to evaluate the outcome of each approach to further enhance the effectiveness of the modality of treatment of ACL tears. AIMS: The aim of this study was to compare and analyze the functional and clinical outcomes of transtibial versus transportal techniques (TPs) in arthroscopic ACL reconstruction using a quadrupled hamstring graft. SETTINGS AND DESIGN: This was a tertiary care hospital and prospective study design. SUBJECTS AND METHODS: Fifty patients with complete ACL tears with or without meniscal injury were included in the study. Patients were then followed up to 1 year and assessed using clinical examination and functional scoring systems. STATISTICAL ANALYSIS USED: Categorical variables were expressed in terms of percentage and proportion. Continuous variables were expressed in terms of mean and standard deviation and data were analyzed using descriptive statistics. RESULTS: The age of the patients included in the study averaged 30.96 ± 7.48. At 1-year follow-up, the average of the International Knee Documentation Committee scoring in transportal and transtibial groups were found to be 84.04 ± 4.64 and 76.72 ± 6.54, respectively. The average Lysholm scoring in transportal and transtibial groups were found to be 83.92 ± 4.82 and 78.56 ± 6.47, respectively. CONCLUSIONS: Significant statistical differences in functional and clinical outcomes were noted among the two groups based on clinical assessment and functional scoring systems at 1-year follow-up. It can be inferred from the study that the TP has a better outcome following reconstruction surgery.
