Abstract: 長期使用を目指したパーシャルデンチャーの 構造と設計Structures and designs of removable partial denture for long-term use Keyword : removable partial denture, long term use, denture design, structural design, prosthetic treatmentA removable partial denture (RPD) with a proper design and fabrication can be expected to maintain the occlusal relationship for a long time without adversely affecting the remaining tissues, even when subjected to repeated loads of severe occlusal forces over many years, and to extend the period of denture use.Conversely, an improperly designed denture can cause the mobility or loss of the remaining teeth, as well as alveolar ridge resorption, which can lead to denture malfunctions and fractures, which can cause repeated instability of the occlusion and the need for dentures to be remade, ultimately ending in occlusal collapse.The process of losing the remaining teeth due to improper prosthetic treatment and occlusal collapse is a negative spiral caused by a chain reaction that ends in an Eichner classification C1 or edentulous jaw.To prevent such situations, we should aim to manufacture dentures that can be used for a long time that protect from further tooth loss.RPD designs must satisfy functional, physiological, and aesthetic conditions.They must be strong, durable, and adapt easily to changes.This article considers the essences of RPD that have been used for more than 50 years.The principles of denture design with the goal of preserving the remaining tissue, structural design to prevent denture fractures, and what dentures should be like to adapt to a super-aging society are also reviewed.適切に設計， 製作された義歯は， 多年にわたる過酷な咬合圧の繰り返し負荷があっても残存諸組織に悪影響を及ぼすことなく， 咬合関係も長期に保全され，義歯使用期間の延伸が期待できる．反対に不適切な設計の義歯は，残存歯の動揺や喪失，顎堤吸 収を惹起し，義歯の不適合や破損を誘発して，咬合の不安定化と義歯の再製作を繰り返し，咬合崩壊に終着していくことが予 想される．不適切な補綴治療により残存歯が喪失し、咬合崩壊へと進む過程は連鎖による負のスパイラルであり，すれ違い咬 合や無歯顎はその終焉と言えるだろう．したがって，咬合崩壊や終焉となる状況を阻止するためには，欠損を拡大させない「長 期間使用できる義歯」の製作を目指すべきである．義歯の設計方針は機能的，生理的，審美的条件を満たしつつ，強度，耐久 性，可変性を成立させなければならない．そこで本稿では，長期間機能するパーシャルデンチャーとはいかなる義歯なのかを， 50 年以上使用されている長寿義歯から学び，残存諸組織の保全を目的とした義歯の設計原則と義歯の破折を防止するための構 造設計を再確認し，超高齢社会に適応する義歯のあるべき姿を考察してみたい． 【 顎 咬 合 誌 4 4 ( 3 ): 3 5 6 -3 6 2 ，2 0 2 5
