Research on the Professional Path and Role of Social Work Intervention for the Elderly with Difficulties—Taking Dongguan Humen’s “Silver Hair Guardian” Project for the Integration of Medical Care and Elderly Care for the Elderly with Special Difficulties as an Example

Abstract: 文章从积极老龄化的视角出发，深入探讨了社会工作介入困难长者的路径研究。积极老龄化是指老年人为了提高生活质量，使健康、参与和保障的机会尽可能获得最佳的过程。笔者首先分析了困难长者的现实需求，包括健康问题、社会参与、心理压力，并结合东莞市虎门镇“银发守护”特殊困难长者医养结合项目实务案例，通过立足于积极老龄化理论，以“健康”“参与”“保障”三大核心要素，设计出“耆享健康”医疗服务、“乐享银龄”养老氛围、“释压舒心”支持体系三大服务路径，旨在提升困难长者的健康水平、社会参与程度与舒缓心理压力。最后，社会工作介入困难长者服务取得了一定的成效，文章对项目服务成效进行了总结，期望有助于积极老龄化的实现。From the perspective of active aging, this paper deeply discusses the path of social work intervention for the elderly. Active ageing refers to the process by which older persons obtain the best possible opportunities for health, participation and security in order to improve their quality of life. The author first analyzes the practical needs of the elderly in difficulty, including health problems, social participation, and psychological pressure, and combines the practical case of the “Silver Guardian” medical care integration project for the elderly with special difficulties in Humen Town, Dongguan City, and designs three service paths: “Elderly Health” medical services, “Happy Silver Age” elderly care atmosphere, and “Stress Relief and Comfort” support system based on the theory of active aging, with the three core elements of “health”, “participation” and “protection”, aiming to improve the health level of the elderly in difficulty. Social participation and psychological stress relief. Finally, the social work intervention in the service of the elderly with difficulties has achieved certain results, and the results of the project service are summarized in this paper, which is expected to contribute to the realization of active aging.
