Abs1166 Effect of Silymarin Against Methotrexate-induced Liver Injury in Rheumatic Diseases, a Pilot Double-blind Randomized Controlled Trial

Year: 2025
Type: conference-abstract
Abstract: Background: Methotrexate (MTX) is a commonly used immunosuppressant for treating rheumatic diseases. It may result in treatment discontinuation due to hepatotoxicity. Silymarin (SMN) is a traditional medicine, with several trials showing its beneficial hepatoprotective properties. However, for MTX-induced hepatotoxicity, its use remains promising. Objectives: To investigate the effect of SMN in the prevention of hepatotoxicity in patients with rheumatoid arthritis (RA) and psoriatic arthritis (PsA) who were initiating treatment with MTX. Methods: The study was a 12-week, single center, double-blind, placebo (PBO)-controlled, randomised pilot trial. Eligible patients were at least 20 years old and diagnosed with active RA or PsA who initiated MTX within 30 days prior randomisation. The participants were randomly assigned in a 1:1 ratio to receive either SMN 140 mg orally three times a day or PBO for 12 weeks. The primary endpoint was the proportion of patients with elevated serum aspartate transaminase (AST) and/or alanine transaminase (ALT) above the upper limit of normal (ULN) throughout the 12-week study period. The secondary endpoints included the elevated serum AST/ALT ≥ 3 times the ULN, the elevated serum AST or ALT > 2 times from baseline, MTX discontinuation, disease activity and adverse events (AEs). As part of this assessment, the planned sample size was 72 participants. The randomised participants were analysed as intention to treat principle. Results: Between 31 January to 30 September 2024, 27 patients were included in the study (RA; n=19, PsA; n=8). The patients were assigned to the SMN group (n=12) and the PBO group (n=12). Both groups exhibited comparable baseline characteristics. The proportion of patients with elevated serum AST/ALT above the ULN was similar between groups (1 versus 1 patient). Nevertheless, one patient in the PBO group had elevated serum AST/ALT ≥ 3 times the ULN and stopped MTX treatment. The SMN group demonstrated a greater reduction in disease activity (the Disease Activity Score-28 with CRP; -2.42 versus -1.54; p-value = 0.14) and lower incidence of AEs (15% versus 21%; p-value = 1.00) compared to the PBO group, although these differences did not reach statistical significance. Conclusion: This pilot study indicated that 12 weeks of SMN intake did not show a hepatoprotective effect against MTX-induced hepatitis. Considering the small sample size, large-scale studies are needed to assess this finding. Trial registration: ClinicalTrials.gov, NCT06277635. REFERENCES: NIL .
