Abstract: The article presents a clinical observation of a 13-year-old child with severe ulcerative colitis and extensive bite wounds of the lower leg, the treatment of which was successfully carried out using an original author's technique, including local application of an alcohol solution of 0.027% chlorhexidine bigluconate without the use of systemic antibacterial drugs. An analysis of the outpatient card of the Children's City Hospital in Novo-shakhtinsk, medical records of our patient from the Scientific and Clinical Center No. 2 of the Federal State Budgetary Scientific Institution "Russian Scientific Center of Surgery named after Academician B.V. Petrovsky" was performed, the effectiveness of this treatment method was demonstrated. The patient's wounds were treated with hydrogen peroxide, then a dressing was performed, applying a band-age with the alcohol solution of chlorhexidine bigluconate 0.027% with the following ratio of components, mass, %: chlorhexidine 0.05% - 50%, rectified ethyl alcohol 95% - 21.1%, distilled water – 28.9%. The dressing was performed up to 7-8 times a day, leaving the bandage until completely dry. In the treatment of patient with the extensive bite wounds , and suffering from a severe somatic disease, it was possible to stop the inflammatory process of the skin and subcutaneous fat by local treatment only, and thereby prevent potential complications arising from the use of systemic antibacterial drugs, such as antibiotic resistance, candidiasis, allergic reactions, ototoxicity, impaired renal and hepatic function, intestinal dysbiosis, and pseudomembranous colitis. At the same time, no chemical burns or tanning of the skin were observed, the procedure was easily tolerated by the patient, did not cause painful sensations, and therefore can be recommended for use.
