Abstract: Pilomatricoma, also known as calcifying epithelioma of Malherbe, is a benign skin tumor originating from the hair matrix cells. Its occurrence in the earlobe is exceptionally rare, often leading to diagnostic challenges. A 30-year-old woman presented to our ENT outpatient clinic with a 3-month history of swelling measuring 3 cm × 4 cm × 2 cm over the left ear lobule. A complete excision was done, and histopathology gave a diagnosis of pilomatricoma. This case highlights the importance of maintaining a broad differential diagnosis when evaluating earlobe masses and underscores the role of histopathological examination for definitive diagnosis. Early recognition and surgical excision remain the cornerstone of effective management, ensuring complete removal and minimizing the risk of recurrence.
