Abstract: requiring fluid resuscitation and inotropic support.Initial investigations showed leukopenia with normal platelet and hemoglobin levels. These results are summarized in Table 1.A diagnosis of neonatal sepsis was made, and she completed a 5-day course of intravenous penicillin and cefotaxime.A blood culture sample taken on the day of birth was sterile. She gradually improved, and inotropic support was weaned off after 72 hours. On day 5 of life, the patient developed worsening respiratory distress on a mechanical ventilator. She was pale, less active, and in severe respiratory distress, requiring an increase in ventilator settings.A chest radiograph was performed and showed worsening patchy opacities in the bilateral midzones and lower zones of the lungs, compared to the initial chest radiograph done prior to the deterioration, suggesting a new onset of pneumonia. IntroductIonPhreatobacter oligotrophus is a newly described genus that was first isolated from ultrapure water in a Hungarian power plant. The bacterium is classified under the genus Phreatobacter within the Alphaproteobacteria class. 1 Oligotrophic bacteria have the ability to grow and proliferate under nutrient-deficient environments, forming biofilms to survive.[2][3][4] The bacterium's presence in hospital water systems and medical equipment can lead to contamination and subsequent infection, particularly in settings where stringent infection control measures are not meticulously followed. It was previously not known to cause infections in humans. However, its ability to survive in extreme conditions renders it a concerning pathogen in clinical settings, and we report a novel incidence of probable Phreatobacter oligotrophu bacteremia in an immunocompromised patient. case descrIptIonA baby girl was born prematurely at 29 weeks via spontaneous vertex delivery (SVD) with a very low birth weight (VLBW) of 1.36 kg.Her mother was a 36-year-old primigravida with primary infertility for >10 years and gestational diabetes mellitus on diet control. She tested positive for group B Streptococcus from a high vaginal swab performed at 27 weeks of gestation.A detailed scan performed at 23 weeks of gestation was unremarkable. The mother presented with contraction pain and rupture of membranes at 29 weeks of gestation. No placental or high vaginal swab samples were taken during the delivery. There was no consanguinity between the mother and father, and there was no family history of primary immunodeficiency disorders.Upon delivery, the patient was born vigorous with good APGAR scores. However, she developed respiratory distress requiring intubation shortly after birth and was admitted to the neonatal intensive care unit (NICU).She was diagnosed with respiratory distress syndrome and received two doses of intratracheal surfactant.Subsequently, she developed hemodynamic instability
