Venous thromboembolism prophylaxis initiation in adults with traumatic brain injury: elusive and hard to pin down
Year: 2025
Type: article
Abstract: Patients with traumatic brain injury (TBI) are at high risk of developing venous thromboembolism (VTE).1 In these patients VTE prophylaxis must be weighed against the risk of intracranial hemorrhage expansions and prevention of VTE. Although the use of VTE prophylaxis has been shown to be beneficial in this population, fears of hemorrhage progression and TBI-related morbidity and mortality have mitigated its early use.2 There is considerable interest among the trauma community to identify the optimal time of initiating prophylaxis. Most centers have individualized protocols on timing and dosing of VTE prophylaxis in patients with TBI. Yet, there is a paucity of strong evidence to support earlier initiation, especially in high-risk patients with severe TBI and those who have had neurosurgical interventions.
