Abstract: Cervical compressive myelopathy is a condition where decreased space in the cervical spinal canal compresses the spinal cord, leading to motor and sensory deficits. We present a case of a 50-year-old male who experienced aggravation of cervical compressive myelopathy after a trivial trauma, resulting in quadriparesis and urinary incontinence. Despite delayed presentation 9 days postinjury, surgical intervention with lateral mass screw fixation and decompressive cervical laminectomy was performed. The patient showed significant improvement, regaining functional independence and bladder continence. This case highlights the importance of considering surgical intervention in delayed cases of cervical compressive myelopathy, even beyond the typical 72-h window. Our experience suggests that surgical intervention can yield better-than-expected outcomes, and we propose considering surgery in delayed cases where patients are willing. This approach may offer improved outcomes and quality of life for patients with cervical compressive myelopathy.
