Abstract: HIV is one of the most commonly seen illnesses in most parts of the world. Constant attention is given on the prevention and containment of HIV transmission primarily by suppressing virus replication using highly active antiretroviral therapy (ART). Increasing numbers of HIV-positive patients on ART or HIV-exposed individuals taking prophylaxis present for surgery and critical care management. Morbidity and mortality have been declining with the aid of these drugs; however, drug-related side effects are to be taken note of. Lipodystrophy is one of the side effects of long-term ART, which can lead to airway-related challenges and we have encountered one such case for which the management we have described here.
