Abstract: A patient presented during a mass casualty event after an airstrike with abdominal injuries from penetrating fragments and the blast wave. The patient was hemodynamically unstable and was transferred to the operating room under wartime triage protocols. Intraoperatively, a grade IV liver injury involving segments IV, V, and VI was controlled by packing. There was a zone I hematoma that was explored, revealing a pancreatic head injury and peri-duodenal hematoma with an intact duodenal wall. A gastric injury was repaired. The patient received five units of packed red blood cells and four units of fresh frozen plasma. The fascia and skin were closed with plans for a second-look laparotomy.
