Clinical examples of pentoxifylline effective use in patients with intermittent claudication and concomitant pathology

Year: 2025
Type: article
Abstract: Patients with lower extremity peripheral arterial disease (PAD), presenting with intermittent claudication, commonly exhibit one or more concomitant comorbidities. In addition to vascular pharmacotherapy — including antiplatelet agents, anticoagulants, lipid-lowering agents, and antihypertensive medications — such patients are frequently obliged to use multiple drugs for the management of their associated conditions. Each pharmacological agent is characterized by its own set of contraindications and is capable of interacting with multiple targets, leading to potential drug-drug interactions The presence of comorbidities in patients referred to vascular surgeons poses significant challenges to the rational selection of pharmacotherapy. Drug selection should therefore be individualized, with careful consideration given to possible drug interactions, the specific organ effects of each medication, and the socioeconomic status of the patient. Among vasoactive agents currently utilized in the management of intermittent claudication, pentoxifylline and cilostazol are the most widely prescribed, exhibiting comparable efficacy and safety profiles. The choice between these agents is determined primarily by the patient's comorbid conditions, current therapeutic regimen, and overall treatment adherence. This article presents a series of clinical cases illustrating the outcomes associated with pentoxifylline administration in comorbid patients with intermittent claudication KEYWORDS: pentoxifylline, intermittent claudication, diabetes mellitus, atrial fibrillation, Crohn's disease, conservative treatment, atherosclerosis, peripheral arterial diseases, lower extremity peripheral artery disease.
