A Case Study of Fuzheng Expectorant Formula Combined with Empagliflozin in the Treatment of Diabetic Nephropathy Stage III

Abstract: 糖尿病肾病(DKD)是糖尿病常见且严重的并发症，尤其在糖尿病肾病III期表现出明显蛋白尿和肾功能减退，治疗难度大。本文结合中医扶正祛邪方与现代西药恩格列净，探讨其联合应用对一例糖尿病肾病III期患者的临床疗效。患者经扶正祛邪方调理脾肾功能、化湿活血，同时联合口服恩格列净控制血糖并减轻肾脏负担，治疗6个月后，尿蛋白显著下降，肌酐水平稳定，临床症状明显改善，生活质量提高。该联合治疗方法安全耐受良好，体现了中西医结合的优势，为糖尿病肾病III期的个体化治疗提供了有效的思路和临床依据。Diabetic nephropathy (DKD) is a common and serious complication of diabetes mellitus, especially in diabetic nephropathy stage III showing obvious proteinuria and renal function decompensation, which is difficult to treat. In this paper, the clinical efficacy of the combination of Chinese medicine Fuzheng Dispel Evil Formula and modern western medicine empagliflozin was investigated in a patient with diabetic nephropathy stage III. The patient was treated with Fuzheng Dispel Evil Formula to regulate spleen and kidney functions, dissolve dampness and activate blood circulation, and at the same time, combined with oral empagliflozin to control blood glucose and reduce the burden on the kidneys. 6 months after the treatment, the urinary protein decreased significantly, the creatinine level stabilized, and the clinical symptoms improved significantly, and the quality of life was improved. The combined treatment method is safe and well tolerated, reflects the advantages of combining Chinese and Western medicine, and provides an effective idea and clinical basis for the individualized treatment of diabetic nephropathy stage III.
