Abstract: Case 45-year-old physiotherapist that was first seen by us in October 2022. History of LASIK in both eyes in Optimax approximately 16 years ago (from the history, for a small myopic prescription; -1 or -2). He was referred with a 2-year history of deteriorating vision in both eyes.On examination he had bilateral post LASIK ectasia but there was also an unusual arc of interface opacity in the superior aspect of the right LASIK flap. The eye was white and quiet with no history of trauma. The patient was given a trial of intensive topical steroids for 1 week which produced no change in the opacity.Since then, the patient has undergone bilateral CXL. I saw him for the first time in September this year. There was possibly some increase in the interface opacity in the right eye but more worryingly, a new interface opacity in the left eye. Both eyes remain white and quiet, and the patient achieves 6/5 vision with a contact lens in both eyes.Though the original plan when first seen was to do a flap lift and culture, it is now 2 years down the line. The opacities do not look infectious and the timeline is not suggestive of it, particularly since it has started spontaneously in the left eye 2 years after the presentation in the right eye. It also does not look like epithelial ingrowth, again the bilaterality and the time since surgery would go against it. The patient is a fit and well man with no known family history of eye conditions. He achieves 6/5 with a contact lens in both eyes and is essentially asymptomatic, so any surgical intervention will risk making things worse for him. However, there has been an increase in the opacity int he right eye, and a new opacity int he left eye since he was first seen.
