Abstract: Background and aim Hypospadias is the most prevalent congenital abnormality of the male genitalia, recurrent hypospadias is a term referrer to patients who have undergone unsuccessful attempts for hypospadias repair due to complications, and the proper management of it is still a therapeutic challenge. This study aimed to evaluate the outcome of two techniques used for the repair of fresh and recurrent hypospadias and to decrease postoperative complications and subsequent recurrence. Patients and methods A total of 40 children were recruited in the study with a mean age of 5 years. Twenty had recurrent hypospadias (group I), and the other 20 had fresh hypospadias (group II). Tubularized incised plate urethroplasty (TIPU) and Onlay Island flap urethroplasty techniques were used for hypospadias repair. Results The recurrent hypospadias group had significantly ( P <0.01) higher hospital stay and operative time than the fresh hypospadias group. No difference was found among groups regarding complications. Although the successful rate of repairs in patients repaired by TIPU was 71.4%, while, in Onlay Island flap urethroplasty was 66.6% yet, we found no significant difference between them. Conclusion The TIPU technique offers a beneficial option for repairing failed hypospadias, it is a well-established technique for recurrent hypospadias that resulted in perfect cosmetic and functional outcomes that are similar to that of other approaches. Proper patient picking is mandatory to achieve satisfactory results. Furthermore, re-repair of hypospadias needs Surgical competencies and should be unique according to the penile status of every single patient.
