Democratic governance and palliative care: Examining the interplay of political systems, economic status, and end-of-life care quality
Year: 2025
Type: article
Abstract: This study examines the disparities in palliative care systems across different countries, focusing on the interplay between GDP, democracy, and their combined impact on the quality of death. Utilizing the Quality of Death Index (QODI) developed by the Economist Intelligence Unit, the research evaluates palliative care quality in 80 countries through 20 indicators across six domains: healthcare environment, human resources, affordability of care, quality of care, and community engagement. The findings highlight that while economic prosperity and democratic governance generally correlate with better palliative care outcomes, their integration significantly enhances end-of-life care. Wealthier countries often lead in palliative care quality due to substantial public healthcare spending and extensive training for medical personnel. However, exceptions exist, such as Uganda, which excels in palliative care despite a modest GDP, and Qatar, which lags despite economic wealth. These anomalies suggest that factors beyond economic wealth, including governmental support and community involvement, play critical roles. The study underscores the importance of fostering democratic governance practices and adopting community-based palliative care models, such as the Neighborhood Network Palliative Care in Kerala, India. Recommendations for policy actions include shifting away from generalized increases in funding and instead promoting context-sensitive policies that prioritize scalable and resource-efficient approaches in low-income countries. Additionally, the study emphasizes the importance of international collaboration and knowledge exchange to disseminate best practices and support the development of sustainable palliative care systems. This research provides a comprehensive perspective on the factors influencing palliative care quality and offers actionable strategies for improving palliative care globally, particularly in low-income and transitioning countries, ultimately benefiting patients and their families.
