Abstract: Objective: Compare the outcomes of the conventional carotid endarterectomy technique with 21 years of experience with the current technique.Material and Methods: We did a retrospective review of Institutional databases and included patients with typical carotid artery occlusion who underwent endarterectomy between 2001 and 2022.Exclusion criteria included patients who previously received treatment for carotid stenosis, underwent endovascular treatment, or had either incomplete imaging workup or incomplete medical records. Results: 76 patients were included in our study, with 56 male patients (73.7%).The mean age was 76 years old.The left carotid artery was more frequently occluded, found in 36 patients (47.4%).There was only one case (1.3%) of carotid occlusion during surgery, which was managed with direct thrombectomy, with no ischemic stroke present after the surgery. Two cases of mortality (2.6%) were present in our cohort. There was a statistical difference between the preoperative mRS and the mRS at discharge(-0.38,CI 95% 0.15-0.68,p=0.001), as well as a difference between the preoperative mRS and the 6 months followup was statistically significant ( -0.802, CI 95% 0.52 ?1.08, p <0.001).Conclusions: According to the results in this manuscript, conclude that the conventional technique has a safety profile, as well as an efficacy and complication rate comparable to the results previously published in the literature.
