Impact of Decreasing Median PELD at Transplant on Children

Abstract: Purpose: Liver medical urgency exception points for pediatric candidates 0-11 years with a PELD score are tied to the median PELD at transplant (MPaT), while adult and pediatric candidates 12-17 years with a MELD score have exception scores tied to the median MELD at transplant (MMaT). MPaT decreased from 35 to 33 in 03/2023. Since MPaT is a single, national value and MMaT is based around each donor hospital, there is concern about a decreasing MPaT leading to a disadvantage for pediatric candidates with a PELD score.
