Genetic Polymorphisms of LILRB3 Drive Monocyte-Mediated Inflammation in Acute Kidney Injury

Abstract: Purpose: In our previous study (Nature Medicine, 2025), we identified a cluster of four missense SNPs in LILRB3 gene causing 167-168EP->PS mutation that are significantly associated with death-censored graft loss (DCGL) in African American kidney transplant recipients. We demonstrated that these SNPs enhance adaptive immune activation and ferroptosis in monocytes, contributing to worse transplant outcomes. However, the underlying mechanisms remain unclear.
