Trends in Molecular Diagnostics for Low Eplet Mismatch Kidney Recipients Managed with Reduced Immunosuppression

Year: 2025

Abstract: Purpose: Eplet mismatch (EMM) has emerged as a biomarker for immune risk in kidney transplant recipients. Retrospective studies suggest that recipients with low EMM may tolerate reduced immunosuppression (IS), but this has yet to be tested in prospective studies. We designed a prospective pilot study to test the safety of early IS reduction in recipients of low EMM deceased-donor kidney transplants (DDKT). We report the results of serial molecular diagnostic testing [donor-derived cell-free DNA (AlloSure®) and gene expression profiling (AlloMap®)] in low EMM recipients on reduced IS, compared to moderate/high (mod/hi) EMM recipients on standard IS.
