Abstract: Purpose: RNA editing is a post-transcriptional mechanism that alters RNA sequences, affecting RNA stability and regulating immune responses. While dysregulation of RNA editing has been linked to diseases like cancer and autoimmune disorders, its role in organ transplantation is still poorly understood. The ADAR enzyme, which mediates RNA editing, modifies double-stranded RNA by converting adenosine (A) to inosine (I), altering RNA structure, and dampening interferon-gamma (IFN-γ) signaling. Given that CIITA is induced by IFN-γ and is the major regulator of HLA class II expression and immune activation in transplantation, we investigated whether ADAR-mediated RNA editing of CIITA is a biomarker for kidney allograft rejection (Fig.
