Tissue-Based Gene Expression Resolves Ambiguity When Histology is Indeterminant in Pediatric Kidney Transplant Recipients

Abstract: Purpose: While Banff classification criteria are the gold standard for histologic evaluation of kidney biopsies, tissue-based gene expressing profiling has the potential to provide clinically relevant information in cases where histology is ambiguous or tissue suboptimal. Varying rates of discrepancy between the molecular and histologic diagnoses have been reported, but reports are limited to adult kidney transplant recipients (KTR). We aimed to quantify agreement and identify factors associated with discrepancy between molecular and histologic biopsy results in a cohort of pediatric KTR with the hypothesis that use of both methods provide added benefit in cases of ambiguity.
