Implementation of a Standardized Immunosuppression Protocol in an Established Kidney Transplant Program Produces Improved Results

Year: 2025
Type: article
Abstract: Purpose: Short and long-term outcomes in kidney transplant recipients (KTR) have improved with advancements in immunosuppressive therapies. Our institution implemented changes to our immunosuppression protocol in September 2023 to optimize dosing of induction and time to attain therapeutic tacrolimus trough levels. Additionally, these changes were intended to decrease our delayed graft function (DGF) and biopsy-proven acute rejection (BPAR) rates. Pertinent protocol changes included lowering anti-thymocyte globulin goal dose to 4.5 mg/kg and initiating de novo weight-based tacrolimus extended-release 0.1 mg/kg on POD 1 for immunosuppression.
