A Modified Tolerance Induction Regimen Incorporating Total Lymphoid Irradiation, Anti-Thymocyte Globulin, and Low-Dose Total Body Irradiation Achieves Sustained Mixed Chimerism and Immunosuppression Withdrawal or Minimization to Mycophenolate Mofetil in Haplotype-Matched Kidney Transplant Recipients

Abstract: Purpose: We previously established a host conditioning regimen combining total lymphoid irradiation and anti-thymocyte globulin (TLI-ATG), which successfully induced mixed chimerism (MC) and immunosuppression withdrawal in recipients of fully HLA-matched kidney and hematopoietic cell transplants. However, in HLA haplotype-matched recipients, MC was immunosuppression-dependent, and withdrawal attempts led to chimerism loss and acute rejection. Here, we present modifications to the TLI-ATG regimen, replacing the final TLI dose with a single very low dose of total body irradiation (svldTBI).
