Abstract: Anomalous left anterior descending coronary artery from the pulmonary artery (ALADAPA; Figure 1) is an exceedingly rare congenital cardiac anomaly with variable presentation, ranging from an asymptomatic murmur to sudden cardiac arrest, and can be a challenging condition to diagnose. In this report, we present the case of a 4-month-old infant with TMCO1 mutation and Cohen syndrome, found to have ALADAPA on screening echocardiography. This case confirms that imaging remains a critical component in the diagnosis and management of ALADAPA and helps provide a blueprint for early detection and treatment of the condition.
