Abstract: Dorsiflexion deformity of the metacarpophalangeal (MCP) joint, a frequent consequence of hand burns, severely compromises both hand function and aesthetics. Traditional surgical techniques, which depend on extensive skin harvesting and grafting with splicing, increase procedural risks, donor site morbidity, and complications such as graft failure, infection, and necrosis. Therefore, it is of particular importance to identify a treatment method that can effectively correct the scar contracture deformity of the hand but also reducing difficulty and risk, improving skin survival rate, and promoting functional recovery. By analyzing the clinical surgical treatment experiences of over ten patients, we have proposed a novel surgical technique: "asymmetric spindle-shaped excision combined with continuous Z skin grafting." This technique maximizes the therapeutic effect by designing personalized scar excision and contracture release, using asymmetric fusiform skin extraction combined with a super tension-reducing suture technique, and continuous Z-shaped skin grafting and functional fixation. This paper thoroughly introduces the surgical steps, technical points, and clinical effects of this treatment. The result indicates that this method can effectively correct the dorsiflexion deformity of the MCP joint after a burn and significantly improve both the function and appearance of the patient's hand.
