Abstract: Extremely premature infants frequently require packed red blood cell (pRBC) transfusions due to the high risk of anemia. Prior research has linked pRBC transfusions to increased pro-inflammatory cytokines and adverse neurodevelopmental outcomes and suggested potential sex-specific differences in inflammatory responses to transfusions. 1 , 2 , 3 , 4 The study by German et al. aimed to explore this potential sex-specific effect and its implications for neurodevelopment. 5 The study showed that a heavier transfusion burden is linked to worse developmental scores in early childhood for both sexes, reinforcing the importance of judicious transfusion practices for all extremely preterm infants.
