Abstract: The transition from surgery to follow-up care is a critical phase in patient recovery, requiring effective nurse-patient communication to identify and address evolving needs. This study examines evidence-based communication strategies that enhance patient understanding, promote adherence to care plans, and reduce complications during this vulnerable period. Key approaches include active listening, the teach-back method, culturally competent communication, and technology-assisted follow-up. Findings suggest that structured communication techniques improve patient outcomes by ensuring accurate needs assessment, reinforcing education, and maintaining continuity of care. Nurses play a pivotal role in facilitating smooth transitions by adapting communication styles to individual patient needs, health literacy levels, and cultural backgrounds. The integration of these strategies into clinical practice can significantly enhance postoperative recovery, reduce hospital readmissions, and improve patient satisfaction.
