Abstract: Medication errors constitute a substantial global challenge, particularly prevalent among patients managing multiple chronic conditions. Medication Therapy Management (MTM) has emerged as an effective, pharmacist-led strategy to mitigate these errors and optimize therapeutic outcomes. Encompassing comprehensive medication reviews, personalized action plans, and continuous followup, MTM offers significant clinical and financial benefits. Evidence consistently demonstrates that MTM improves medication adherence, reduces adverse drug events (ADEs), and lowers healthcare costs, especially in the management of chronic diseases such as diabetes mellitus, cardiovascular disease, and hypertension. Pharmacists play a pivotal role in identifying drug-related problems, enhancing disease control, and promoting patient satisfaction. The increasing global burden and associated costs of chronic illness underscore the imperative to expand pharmacist-provided MTM services within interdisciplinary care teams. This review highlights the benefits of MTM in enhancing health outcomes and delivering costeffective care
