Abstract: Embedded systems can be used for real-time monitoring and assessment of cephalosporin concentrations in children’s urine samples, which is crucial for rapid response and clinical decision-making. To establish a rapid and sensitive ultra-high performance, liquid chromatography-tandem mass spectrometry (UPLC-Q Orbitrap HRMS) method was integrated with embedded medical intelligent systems for the screening of 12 types of cephalosporin antibiotics in children’s urine samples. This integration aims to leverage the high efficiency and automation capabilities of embedded systems to enhance the detection process. Urine samples were processed with phosphate buffer and HLB solid-phase extraction before separation on a Hypersil Gold C18 column. Quantification used matrix-matched standards and external calibration, with embedded systems ensuring process control and reproducibility. The method showed good linearity ([Formula: see text]), detection limits from 1.40–13.51[Formula: see text][Formula: see text]g/L, and quantification limits from 4.44–43.03[Formula: see text][Formula: see text]g/L. Recovery rates were 60.66–113.39%, and precision was 1.98–13.58%. Cephalosporin concentrations in 50 urine samples ranged from 4.904[Formula: see text][Formula: see text]g/L to 125.862[Formula: see text][Formula: see text]g/L, suggesting exposure risks. This integrated approach offers a simple, sensitive, and precise method for detecting cephalosporins in children’s urine, utilizing embedded systems to automate analytical processes, aligning with modern smart healthcare initiatives.
