Abstract: - Scaled manuscript submissions from paper mills and misuse of generative AI represent challenges for journals, publishers and readers. - Paper mills are likely to manipulate peer review as their rate-limiting step towards publication. - The different stages of peer review can be reconsidered using the Swiss Cheese Model of error and accident causation and prevention. - Simple changes to peer review processes could improve the detection of unethical, derivative and low-value manuscripts.
