Abstract: Tibial spine fractures are uncommon, particularly in the pediatric population. Multiple case reports have described the treatment of tibial spine fractures in pediatric patients as it presents unique anatomical challenges. Displaced tibial spine fractures can lead to instability and become entrapped in the knee, resulting in decreased range of motion and knee contractures. This case highlights the unique treatment complications of severe knee contracture coinciding with femur deformity after an iatrogenic physeal fracture sustained during manipulation under anesthesia following a tibial spine repair. Key Concepts: (1)Arthrofibrosis is a common following tibial spine repairs in the pediatric population.(2)To prevent contracture and arthrofibrosis, rehabilitation should be started as soon as immobilization ends.(3)Iatrogenic femur fractures can occur during manipulation under anesthesia.(4)Sagittal plane remodeling in the femur can occur in patients with significant growth remaining.
