Abstract: This review highlights significant advancements over the past decade, showcasing various macrocyclic drug candidates demonstrating improved potency and selectivity compared to traditional linear compounds. Macrocycles face complex and resource-intensive synthesis challenges despite their therapeutic potential, which can hinder rapid development cycles. Furthermore, achieving desired conformational stability is essential for optimizing efficacy and bioavailability. Various macrocycles, including natural products like porphyrins and macrolide antibiotics, are explored alongside synthetic variants such as calixarenes and cyclodextrins. While some macrocycles serve as direct therapeutic agents-like macrolide antibiotics for bacterial infections-others are integral to drug delivery systems. Overall, this review emphasizes the evolving landscape of macrocycle applications in both therapeutic and diagnostic contexts for bone health, advocating for continued refinement of synthesis strategies to fully exploit their potential.
