Systemic Therapy of Unresectable Non-functioning Pancreatic Neuroendocrine Tumor: Focusing on Grade 1–2

Year: 2025
Type: article
Abstract: Grade 1-2 pancreatic neuroendocrine tumors represent a heterogeneous group of neoplasms with variable biologic behavior and multiple therapeutic options.Individualized treatment strategies based on tumor grade, burden, symptoms, and somatostatin receptor (SSTR) expression status are the need of the hour.Somatostatin analogs such as octreotide or lanreotide are the standard initial therapies for treating most SSTR-positive patients and are known to offer both symptom control and tumor stabilization. For patients with progressive disease or higher tumor burden, peptide receptor radionuclide therapy with 177 Lu-dotatate is effective. For patients with progressive disease, molecularly targeted agents such as everolimus and sunitinib are approved options and have been proven to be efficacious. An important option for patients with high tumor burden or aggressive features is cytotoxic chemotherapy, particularly capecitabine plus temozolomide.Optimal sequencing of these agents has not been clarified, and further studies are needed to personalize systemic therapy. This review summarizes current evidencebased strategies for systemic treatment of grade 1-2 pancreatic neuroendocrine tumors, based on the current guidelines and clinical data.
