Abstract: Objective: To report a rare case of gastric lipoma presenting with intussusception, highlighting its diagnostic and management challenges. Case: A 30-year-old female with no prior medical history presented with chronic abdominal pain, vomiting, and diarrhoea for one year. Physical examination revealed a palpable epigastric mass. A computed tomography scan demonstrated a 4 × 8 × 10 cm gastric lipoma causing intussusception of the antrum into the duodenum. Oesophagogastroduodenoscopy showed normal gastric mucosa but failed to navigate the pylorus. Exploratory laparotomy confirmed a pedunculated mass arising from the posterior gastric wall, which was resected with a 1 cm safety margin. Histopathology confirmed a benign submucosal lipoma with mature adipocytes and no atypia. The patient had an uneventful recovery. Conclusion: This case underscores the rare presentation of gastric lipoma with intussusception and the diagnostic utility of computed tomography imaging. Surgical resection remains the mainstay for symptomatic cases, while further research is needed to optimise management strategies for asymptomatic lesions.
